Who is eligible for the Ontario Health Insurance Plan (OHIP)?
You are eligible for OHIP if:
- you are a foreign worker who holds a work permit valid for at least six months that allows you to work in Canada, and you also have a formal agreement in place to work full-time for an employer situated in Ontario which sets out the employer’s name, your occupation, and states that you will be working for no less than six consecutive months or,
- you are the spouse, same sex partner, or dependent child (under 22 years old or 22 years and over if dependent due to a mental or physical disability) of an OHIP-eligible foreign worker
More OHIP information is available at: http://www.health.gov.on.ca/english/public/pub/ohip/temp_foreign_workers.html
When will I need to submit a claim form?
If the health care provider does not accept UHIP as a valid means of coverage for services acquired, you will be billed out-of-pocket. In this situation you must keep all receipts of services acquired, fill out a claim form and mail these items in to the Sun Life Assurance Company of Canada. For access to claim forms and mailing information please refer to the following website: http://www.uhip.ca/_uploads/en_ClaimForm.pdf
Which Health Care Providers accept UHIP as a valid means of coverage?
The following website link provides a complete list of hospitals across Ontario that will accept UHIP as a valid means of coverage without the UHIP member incurring any out-of-pocket charges for covered services: http://www.uhip.ca/_uploads/en_PreferredHospitalNetwork.htm
Why does my UHIP card expire August 31 when I’ve purchased coverage beyond this date?
The University Health Insurance Plan has been created for an academic session in which coverage is effective from September 1 to August 31. As a result, anyone purchasing UHIP coverage for three months or more after June 1 will receive an initial UHIP card with an expiry date of August 31. This initial card in no way indicates ineffective coverage beyond that date; instead, it confirms coverage from the date of purchase through to August 31. The PGME office will make certain that coverage purchased from September 1 onward be renewed, and that a new UHIP card reflecting this update be mailed to you.
